Hazard Identification, Risk Assessment And Risk Control (HIRARC)
Introduction
HIRARC consist of three activities- Hazard Identification
- Risk Assesment
- Risk Control
It a systematic measurable to accessing hazard and their associated risk as well as provide a method to control the risk.
- Hazard = a danger or risk/a potential source of danger.
- Risk = a situation involving exposure to danger.
- Danger =the possibility of suffering harm or injury.
Hazard
A common way to classify hazards is by category:
- biological - bacteria, viruses, insects, plants, birds, animals, and humans, etc.,
- chemical - depends on the physical, chemical and toxic properties of the chemical,
- ergonomic - repetitive movements, improper set up of workstation, etc.,
- physical - radiation, magnetic fields, pressure extremes (high pressure or vacuum), noise, etc.,
- psychosocial - stress, violence, etc.,
- safety - slipping/tripping hazards, inappropriate machine guarding, equipment malfunctions or breakdowns.
Risk Assesment
Risk assessment is the process where you:
Identify hazards and risk factors that have the potential to cause harm (hazard identification).
Analyze and evaluate the risk associated with that hazard (risk analysis, and risk evaluation).
Determine appropriate ways to eliminate the hazard, or control the risk when the hazard cannot be eliminated (risk control).
The OSH Answers document on Risk Assessment has details on how to conduct an assessment and establish priorities.
Risk Control
Human behavior is often the focus of safety investigations. Managers and others are primarily interested in how to get people to do the right thing at the right time. Attention is generally given to risky actions taken by people.
Human Behavior: The Main Cause of Risk?
Human behavior is often the focus of an incident investigation. What did someone do (or not do) that contributed to the outcome of the event? Many solution recommendations attempt to control human behavior in one manner or another. Recommendations such as writing better procedures, sending people to additional training, and implementing best work practices abound
Hierarchy of control
The Hierarchy breaks down as follows, with the most effective measures at the top of the pyramid and the least effective at the bottom.
- Elimination/substitution. At the very top is the best way to deal with a safety hazard, which is to eliminate it altogether by preventing exposure to the hazard before it even occurs. In substitution, you seek to permanently reduce the risk by substitute a less hazardous material or reduction of system energy. These are process design solutions that require a permanent change to how a job is performed.
- Engineering controls. Change the structure of the work area to reduce exposure using safety devices or barriers. An example would be to place a high fence around a dangerous location to prevent access.
- Administrative & work practice controls. Implement procedures that require workers to do things to reduce their exposure to a risk. A lockout/tagout program is an example of an administrative control. Set expectations that workers will engage in safe work practices. Another example is the use of warning signs, sirens and alarms.
- Personal protective equipment (PPE). Make sure employees wear the proper protective clothing, gloves and eyeglasses for the job. Examples are safety goggles, respirators, fall protection and hearing protection.

Assalamualaikum kami dari kumpulan 3. Pada minggu tersebut iaitu 7/8/2018 kami telah memaparkan hasil video yang dilakukan setiap kumpulan yang bertema KESELAMATAN DAN KESIHATAN. Dalam penghasilan video setiap kumpulan dapat menunjukan bakat terpendam yang ada dalam diri mereka dari segi percakapan dan lakonan. Malahan pula dari penghasilan video ini kami dapat memupuk nilai kerjasama diantara semua ahli kumpulan.

1.What is an OHS Management System?
- An Occupational Health and Management System (OHSMS) is a set of plans, actions and procedures that assist an organization to systematically manage health and safety risk associated with their business.
- The overall aim of an OHSMS is to continuously improve OHS performance through the effective management of OHS risks and activities in the workplace.
OCCUPATIONAL SAFETY AND HEALTH ACT 1994 (ACT 514)
PART VII SECTION 30
- Every employer shall establish a safety and health committee at the place of work in accordance with this section if :
a)there are forty or more persons employed at the place of work;
or
b) the Director General directs the establishment of such a committee at the place of work.

Composition of Safety and Health Committee
- A safety and health committee must consist of a chairman, a secretary, representatives of employer and representatives of employees.
- Under the regulations the employer or his authorized manager shall be the chairman of the safety and health committee.
- Where 100 persons or less are employed, there should be at least 2 representatives each from employer and employees.
- Where more than 100 persons are employed, there should be at least 4 representatives each from employer and employee.
Functions of Safety and Health Committee
- Assisting in the development of safety and health rules and safe system of work and reviewing the effectiveness of safety and health programs
- To conduct investigation on any matters on safety or health and to bring it to the attention of the employers.
- To carry out studies on the trends of accidents, near-miss accidents, dangerous occurrences, occupational poisoning and diseases which occurs at the place of work.
- To inspect the place of workplace at least once in every three months.
- Report to the employer of any unsafe or unhealthy condition or practices at the place of work with recommendations for remedial action.

Safety Training
- Training is an integral part of Occupational Safety & Health (OSH).
- To ensure the success of any OSH program at the workplace, adequate and effective training must be implemented for all those responsible for OSH.
- Training enables managers, supervisors and workers to understand the workings of safety management systems and the legal compliance required.
- They will then understand their own responsibilities and the necessary actions to be taken towards upgrading safety and health at their respective workplaces.

a) Supplier of the material, substance or equipment;
b) DOSH
c) Accredited Training Providerd) Health Departments;

What is OSH.
- Occupational safety and Health is a condition where an employee needs a safe and healthy working environment for himself and others who may be affected by his / her acivities
Osha History.
1) Humurity Code (REFUND)
- Trecked in Babylon as early as 2500 BC.
- In that state a ruler reigns in accordance with the code of low.
- Determining accident prevention is done through retaliation through data.
2) United Kingdom
- Since the 19th century the industrial revolution began.
- Always work with machines instead of hands and animals.
Osha Role
Since the 12th year of the late 19th century.
- Boilers before 1914
- Safety machinery 1914-1962
- Industry 1953-1967
- All industries 1970-1994
- All fields of employment after 1994

The Occupational Health and Safety Act 1994 is based on six principles :
